Do not force an incompletely seated aligner or cut a broken, sharp one at home. If lost, do not independently switch to an earlier or later aligner. Report the stage and problem; continuation depends on your treatment position.
This guide explains information to prepare for three common problems and urgent assessment signs. For normal use and care, see the treatment guide and cleaning guide.
Remove it and check stage markings against the prescribed sequence. Check that orientation is correct, no obvious dirt or crack is present and no new obstruction is felt on a tooth. With clean teeth and appliance, report persistent visible gaps, new marked pressure or repeated poor fit, providing a photograph.
Report several days of missed wear or a lost attachment too. Multiple causes are possible; a photograph alone cannot establish that teeth are not following treatment. Do not force seating, reshape with hot water or move early to the next stage. The clinician assesses appliance, attachments and movement.
Gripping after a stage change differs from part remaining away from teeth. Describe the area, pain or gap and whether the previous aligner showed it. Report a detached attachment separately, without assuming every gap has that cause or reflects under-wear.
Some patients receive specific seating aids or techniques. Using an unprescribed online tool or biting harder to overcome clear poor fit is different. Clarify the limits of your instructions; general seating advice is not an automatic answer for painful, broken or markedly unseated appliances.
Note crack location and whether the appliance remains whole. Do not force continued wear if a sharp edge irritates tissue. Do not glue, cut or hot-water reshape it. Photograph damage, keep pieces in the case if possible and try to contact the team the same day.
Small marks do not all mean the same thing, but a crack may alter planned tooth engagement. The clinician considers location and stage and advises review timing. Do not independently choose the previous aligner, next aligner or a period without one.
Length alone does not decide. Location, sharpness, shape and seating also matter. Describe onset and accompanying symptoms without needing an exact damage label. Do not open and close a crack or flex the appliance as a strength test.
Report the irritation location, pain severity and effect on wear. Examination may be needed if photographs or messages are insufficient. The AAO appliance-problem guide also advises against continuing to wear a sharp, painful or badly distorted aligner.
Briefly check the last removal location, case, tissues, trays and bag. If missing, report number, days of wear and available earlier or later aligners. Having another appliance does not mean it should be inserted immediately.
The next stage used depends on current positions and planned change timing. The clinic may advise replacement, another stage or review. Do not apply a universal online instruction to move on. Concealing interruption makes suitable planning harder.
Active aligners represent different movement stages, not duplicate spares. The AAO treatment guide recommends promptly contacting your orthodontist about a lost or broken aligner and asking how to continue the sequence.
If found later, describe its condition and any other aligner worn meanwhile. Clarify current instructions rather than silently resume the old plan. This guide concerns active sequences; post-treatment retainer loss has a different purpose, explained in the lost retainer guide. Similar clear appearance does not establish equal function.
State stage, onset, exact poor-fit or breakage location, and pain, sores or a lost attachment. Add clear appliance and seating photographs where appropriate. For loss, give last wear and available appliances.
Do not change sequence until instructed. If advice is unclear, ask which aligner, for how long and when to attend. Persistent visible poor fit may require examination rather than messages alone.
Breathing or swallowing difficulty, severe facial or jaw trauma or uncontrolled bleeding requires emergency help without waiting for a routine message. If you suspect you have swallowed or inhaled a broken fragment, promptly seek professional assessment rather than wait only for a replacement-aligner appointment. The Guy’s and St Thomas’ NHS guide distinguishes swallowed dental objects from objects entering the airway. Affected breathing is an emergency; the healthcare team decides which assessment is needed.
A sharp-edge wound, marked pain or another issue may not need emergency services but may warrant same-day clinic contact. Severity cannot be classified definitively remotely. Seek professional help when unsure and follow individual advice.
Use the case whenever removed, not tables, tissues or open bags. Clean appliance and teeth appropriately after meals and reinsert. Record change dates and reviews; knowing the stage speeds communication. Check initial seating as demonstrated and promptly report clear gaps.
These steps cannot prevent every problem; poor fit and breakage still need assessment. The cleaning guide covers daily care and the treatment guide covers process and suitability limits.
