Right to make a request

You may submit a request to exercise your rights concerning your personal data under Article 11 of Turkey’s Personal Data Protection Law No. 6698.

Applicant information

Your request must include the following information:

• Name and surname.
• Turkish identity number, or passport/identity number for foreign nationals.
• Postal address or email address for correspondence.
• Telephone number.
• Subject and explanation of the request.
• Any supporting documents.

How to apply

You may submit your request to the clinic in writing or send it from your registered contact details to drhaydemirdr@gmail.com.

Response time

Requests are concluded as soon as possible, depending on their nature, and within thirty days at the latest. If the procedure entails an additional cost, a fee may be charged according to the tariff set by the Personal Data Protection Board.
